I've got some photo-realist bums on my bedroom wall and it pleases me immensely to think of them being lovingly and pain-stakingly painted. Photo-realist cross-stitch just magnifies the dedication required and, for me, that's what makes this so powerful - for the artist to have undertaken to live with such disturbing subject matter for such a length of time is remarkable and speaks of a strong motivational force.
